Why do we do what we do? 

Daisy Pawlowski, MSN, FNP-BC, Owner and Primary Care Provider at HolistiCare LLC, wanted to bridge the gap in health care. She combined her Home Health Care and Primary Care experience and created this Company. 

What you can expect from us is personalized, holistic, comprehensive care. You will get the time you deserve with the Provider to discuss your healthcare concerns. We offer office visits, tele-health visits, and home visits (for homebound patients). We provide a wide variety of services ranging from Primary care, to work physicals, to telehealth urgent care visits, to wound care and so much more.
